I am facing problem that, when I click on Add button in BOTranslation dialog, the form just disappear. What could cause it? My child form is assigned correctly.

The type resolution service may be throwing an exception and that exception is being snuffed by Visual Studio.  Make sure that your solution is completely rebuilt and if it still fails, try restarting Visual Studio.
